Stroke Nurse Follow up post discharge for inpatients and outpatients
As part of a patient’s ongoing care, we offer a follow up Stroke Nurse Service which is designed to support all patients in NHS Highland during their recovery and help reduce the risk of further strokes and TIAs. We work collaboratively with Chest Heart & Stroke Scotland (CHSS) and patients may be contacted by a Stroke Nurse from CHSS, who also help deliver the Stroke Nurse Service in NHS Highland. The follow up time is between 6-8 weeks, however this can vary. We can be contacted on 01463 704086/07484078569 or on email nhsh.strokespecialistnurses@nhs.scot
